2. Submit an Online Referral Form

For a fast and secure referral experience, you can refer a patient online through our HIPAA-compliant referral form:

• Click the Refer a Patient button to access the patient referral form.
• Enter your phone number or email to see if you are in our database. If you’re new, we’ll ask for your contact information.
• Provide basic information about the patient to start the referral process.
• Our care team will contact the patient directly to conduct a pre-admission interview and schedule their intake visit at a BHG treatment center of their choice.

3. Direct Walk-In Access

Patients are welcome to walk into any BHG treatment center for intake and medication-assisted treatment.
